Here you may find paperwork your therapist requests or update your contact information. Any specific forms (updating insurance or payment authorization) may be requested by contacting your provider at their email address or phone number. You may also request a password reset from Rebecca at rebecca@openheartstherapy.com. Please note, if a reset password email is received you’ll have 24hrs to respond before it expires.
